Yes, TomTom Traffic only works in Europe on a European device like this one. The European maps are pre-installed. Additional maps from around the world, as well as the American maps, can be downloaded and installed free of charge from the manufacturer's homepage.
I don't quite understand the question, as the SIM has nothing to do with the cards. I'll try to answer anyway.
Regarding the cards, they must first be downloaded and installed on a PC. The device usually only has space for one card, which can be deleted and exchanged. If necessary, buy an additional microSD, then you can also install several cards.
