The loudspeaker may be defective if someone at the other end is listening quietly. Volume is at maximum or is it just the handset or does it need a new system?

The volume at the other end depends on the microphone when using the handset. The holes in front of the microphone may be clogged. These can be cleared of dirt with a needle.
As an alternative to the internal microphone, you can also use a Bluetooth speaker with microphone, Bluetooth headset or the smartphone. This would certainly be helpful for testing the volume at the other end so that it can be ruled out that it is the other end.
I myself use the phone in most cases with a conference speaker (Jabra Speak 710) connected via Bluetooth. This allows me to make calls hands-free and without "head strain".
