The onset of the disease and the progression of hidden lesions are gradual, relatively slow, and difficult to define until surface cavitation occurs. The devices DIAGNOdent and DIFOTI can be helpful in identifying early and hidden carious lesions. The goal of minimally invasive operative dentistry is to maximize the preservation of healthy tooth structure in order to maintain a healthy dentition for a lifetime. More emphasis must now be placed on assessing caries risk, placing patients in a low caries risk status, remineralizing non-cavitated lesions using remineralization pastes, abandoning the surgical approach to caries treatment, and repairing defective restorations instead of replacing them.
