This is the sequence in the Structured Information Collection (SIS®). However, this self-assessment presents new challenges: What to do if the person in need of care cannot provide information? How is a conversation documented quickly and understandably? And what about expert standards, risk assessment, and documentation? The 3rd updated edition has been aligned with the revised and currently available expert standards. The relevant screening or assessment for the SIS® has been updated or added. The focus is also on aligning with the assessment instrument. Additionally, the changed form of quality assessment has been taken into account: with a comparison of the responsibilities related to the SIS® for nursing professionals and nursing management.
