The book aims to make an article on the social debate on dealing with the last phase of life and on the professionalisation of the culture of care. It gives suggestions on how the concept of health care planning (HCP) can be introduced and integrated in residential care facilities for the elderly and with which attitude the residents can be advised and accompanied in planning their last phase of life. The discussion offer is carried out by trained discussion facilitators and is intended to support residents in developing their treatment wishes and preferences for the case of incapacity and to record them in writing.
