Die Prolaktin-Adenome

This was a retrospective, descriptive, and analytical study of 77 cases of prolactinomas collected and followed between 2000 and 2017 in the Department of Endocrinology and Diabetology at CHU Hedi Chaker in Sfax. Various clinical, paraclinical, and therapeutic characteristics were described. Our patients were divided into three groups based on the size of their prolactinomas. Statistical correlations between tumor size and clinical and biological parameters were sought. Prolactinomas were studied in pregnant women, older individuals, and those discovered incidentally. The average age of our patients was 38.3 ± 14.2 years. They were comprised of 51 women (66.2%) and 26 men (33.7%). The average diagnostic latency was 47.37 months. The pituitary tumor syndrome was reported as the reason for discovery in 62.3% of cases. The endocrine syndrome of hyperprolactinemia manifested in women as menstrual cycle disturbances (78.4%) and galactorrhea (70.5%). In men, the clinical picture was dominated by erectile dysfunction (42.3%) and/or decreased libido (34.6%). The syndrome of anterior pituitary insufficiency was observed in 75.3% of cases. The number of anterior pituitary deficits correlated significantly with tumor size. When comparing the three groups, we found that age, CDD, metabolic parameters, hypopituitarism, and pituitary enlargement on imaging were significantly different.
