This book introduces a paradigm shift in nursing: With the 4-phase model, nursing finally receives the right status and a contemporary structure. After all, it is the nursing staff who do the main work. Every geriatric care facility can use the 4-phase model. It is based on the AEDL, the nursing case histories and the usual nursing care planning. However, it works with new forms such as day care plan and nursing report. Instead of laboriously signing off service complexes, this model only documents the deviations. This simplifies the paperwork and makes it possible to react quickly to changes in care. Employees and residents are more satisfied. And the new model also stands up to audits by the MDK or home supervisory authorities.
