The dialectical-behavioral therapy for borderline and comorbid substance use disorders (DBT-S) describes a therapeutic approach that aims to treat both disorders equally from the outset. The goal is to build and improve skills for emotion regulation while simultaneously reducing substance use or achieving and maintaining abstinence. The practice guide presents DBT-S-specific treatment strategies, explains the structure of a DBT-S skills program, and illustrates the concrete application of DBT-S in various treatment settings.
The book first provides information about the two disorder profiles, the effects of comorbidity, and their development. It also discusses the evolution of DBT-S and references results from efficacy studies. Based on standard DBT, specific extensions of DBT-S and their treatment strategies are introduced. These include special "attachment strategies" to help patients establish a bond with therapy and prevent therapy dropouts. The work within the consultation team, the concept of "dialectical abstinence," and strategies for dealing with ongoing or relapsing substance use are explained. Numerous informational and working sheets are provided for teaching skills in managing cravings, the consequences of years of use, and building and reinforcing abstinence. The possibilities for implementing DBT-S in inpatient, partial hospitalization, and outpatient settings, as well as in social work, are outlined with concrete processes and examples.
