Does this device transmit from the third floor in an apartment building all the way down to the basement?

This depends very much on the house or how thick the walls are. Outdoors it transmits very far and in the house we have only tested it for one floor. Over four floors, however, I don't think you will find a standard baby monitor that can do this. I would use an app for two smartphones or a baby monitor for via WLAN or mains power.
