Unfortunately, we do not have any personal user experience with this product, so unfortunately we have no information as to why a large number of these headsets have been returned.
Mine has been working very well for 2 years and I'm very happy with it. It's better than anything else I've tried with gaming functionality. To do better, you'd need a hi-fi headset, but then you'd need an external sound mixer to separate 'chat/game' and an external microphone. The top strap can be adjusted by screwing and unscrewing, but at the same time, once it's properly adjusted, why would I need to change it? Admittedly it's rather heavy (500g) but I've got used to it and it suits me fine. Every time I try a logitec or razer or other, I run back to the Maxwell, it's really superior, both in terms of listening and the quality of the microphone.
The problem you mentioned could be due to the microphone settings in COD. Try these settings: First open the game and go to the 'Settings' menu. There you will find the 'Audio and Graphics' tab. Click on it and you will see the options for your microphone. Here you can adjust the microphone level, switch it on or off and specify who should hear you - everyone, only friends or nobody. Also check the Audeze settings: https://www.audeze.com/pages/maxwell-user-guide#:~:text=Maxwell%20is%20compatible%20on%20PS4,port%20using%20the%20included%20adapter.
